Important Information:

Please note the 2012 Pop Warner Physical Form is the ONLY form that will be accepted. Although the form has a place for an RN to provide a physical in Florida Pop Warner will not accept a physical completed and signed off on by a RN.

An original birth certificate will be needed for our records. Once we have a certified copy in our books your birth certificate will be returned to you.

Your child’s latest reposrt card must be turned in along with the above paper work to have a completed registration. These form can be handed in at any of our pre-season in person registrations or at the first practice.  Your child WILL NOT be permitted to take the field until ALL paperwork is completed.

Season begins August 1, 2012, we hope to see you there.
